L&I Claim

Lauren Mallory, LICSW, CCTP, is an approved Treating Therapist for L&I Claims. If you have an L&I Claim and would like to be seen, please ensure you have the following:

1. Claim Documentation – A statement from the State or City (self-insured) confirming an accepted claim, including your claim number and the accepted conditions for treatment.

2. Attending Provider – Identify your Attending Physician or ARNP. (Please note: EPIC Possibilities does not perform evaluations.)

3. Screening Form – Complete the screening form on the Contact Page, using your claim number in the insurance line. In the body of the email, confirm that you have all of the above documentation.
